Transition

The Transition Program at Rivendell School is a key component of the educational support provided throughout every student's enrolment at Rivendell School. Designed to ensure continuity of learning and support beyond the school setting, the program faciliates gradual, individualised transitions into the most appropriate educational, vocational or post school environment.

Transition planning begins at enrolment, with staff working closley with students, families, home schools, and external agencies to identify pathways. Students are supported to set goals in the 5 domains of transition. These include, education, employment, independent living, personal and social relationships and community involvement. Students partcipate in targeted transition activites, including trial attendance at future settings, development of tailored support plans, work exploration and experience. Adjustments and supports are scaffolded to match each student's cognitive, emotional, and social readiness.

Whether students return to mainstream schooling, distance education, specialised programs, or work placements, the transition program ensures they are equipped with the skills, confidence, and resources needed for a successful reintegration.
